Phone Consultation
For patients who are unable to visit our office, we provide the option of a phone consultation with Dr. Fred Grossman.
The patient is required to fill out the patient history form Part 1 and Part 2 on our website and either e-mail info@malecarecenter.com it or fax (303-571-1520) it back to our office prior to the consultation. Dr. Grossman will review the patient’s history form and interview the patient by phone at the scheduled time.
A written report including an assessment of the patient's condition as well as recommendations for further management will be mailed to the patient within the next ten days. Most men will benefit from this service whether or not they proceed with treatment or procedure with Dr. Fred Grossman.
The Initial consultation lasts for 30 minutes and is at a cost of $250.00 (not reimbursable by any insurance).
The Follow Up consultation lasts for 15 mins and is at cost of $100.00 (not reimbursable by any insurance).
